Time is running out to Complete your Chicago Lives Healthy Well-Being Assessment (WB5)

A reminder letter is going out this week for those who have not done their Well Being 5 (WB5) assessment (data as of February 16, 2017).  This is the computer based questionnaire that will take less than 15 minutes to complete.  Everyone who is a participant in the Chicago Lives Healthy wellness program needs to get it done.  The deadline for completion is February 28, 2017.

There is now a reminder in place for those employees who have access to the City system.  The reminder pops-up when they sign on.

You can help by reminding people as well.

Healthway’s records indicate that as of February 16, 2017, you have not yet completed your Well-Being Assessment® (WB5) for the Chicago Lives Healthy wellness program. The deadline to complete your WB5 is February 28, 2017. This deadline will not be extended.

The WB5 is a simple, confidential questionnaire that looks at your lifestyle habits and current health to see what may be helping or hurting your overall well-being. Simply answer a few focused questions and get instant feedback in your Well-Being Report, including suggestions on ways to increase your well-being. All questions are simple to understand and easy to answer. It will take about 15 minutes to complete the questionnaire.

What if I don’t complete the WB5? If you do not complete the WB5 requirement by the February 28, 2017 deadline, the City of Chicago employee will incur a $50 per non-participant increase in his/her monthly health care contribution.
