Use your City of Chicago pharmacy benefit to obtain at-home COVID-19 test kits without a prescription
On January 10, 2022, the Biden administration finalized guidance for providing no-cost, at-home COVID-19 antigen tests through commercial and employer-offered health insurance plans. Covered individuals can receive up to 8 tests per 30 calendar days. The test maximum does not include any PCR tests ordered or administered by a health care professional.
City of Chicago employees and their covered dependents, who are enrolled in health benefits, will be able to obtain at-home tests through their CVS Caremark prescription drug benefit. Members have two coverage options:
- Show your CVS Caremark ID card at any in-network pharmacy counter (CVS, Walgreens, and most other pharmacies): Members will pay $0 for at-home COVID-19 antigen tests at any in-network pharmacy that has COVID-19 antigen tests available for purchase.
- Reimbursement for tests purchased starting January 15, 2022: Members may upload a copy of their receipt to Caremark.com to submit for reimbursement or submit a paper claim.
Members will be reimbursed no less than the actual price, or $12 per test (whichever is lower).
Members are required to submit their demographics, the number of tests, the type of test purchased, where it was purchased from, and a copy of the purchase receipt dated from Jan 15, 2022 onwards, and attest that the OTC kit is for personal use.
At this time, CVS Caremark will not be offering mail-order COVID-19 tests as part of the Mail Order pharmacy benefits.
